Teri Diederich, DNP, APRN-NP, FHFSA was named the 2024 HFSA Nursing Clinical Excellence Leadership Award winner.
Teri Diederich, DNP is the Cardiology APP supervisor at Nebraska Medicine and supervises 22 APPs. Dr. Diederich has been a heart failure and transplant nurse practitioner at Nebraska Medicine for 16 years. At Nebraska Medicine, Teri led efforts to develop the HF-Optimize clinic, an APP led, multidisciplinary clinic aimed to improving GDMT use for heart failure patients. In 2022 she helped to update the protocols to add a HFpEF pathway utilizing the updated heart failure guidelines. Since inception, the HF-Optimize clinic has had over 1300 referrals and demonstrated significant positive patient outcomes that have been published by Teri as the lead author in the Journal of Heart & Lung. Teri oversees and assumes responsibility for the daily management of patients with CardioMEMs. Teri has developed numerous HF education modules and webinars for newer nurses and advanced practice providers at the local, regional, and national level.
Teri is an active member of HFSA having served on the education committee, ASM planning committee twice, nursing committee and currently the HF-OMT committee. Teri became a HFSA Fellow in 2021. She has received her CHFN through AAHFN in 2016 and her HF-CERT through HFSA in 2023. Teri was recognized by the Nebraska March of Dimes in 2019 with the “Excellence in Advanced Practice” award and by Nebraska Nurse Practitioners in 2022 as the “Clinician of the Year”.
