Survey Overview & Purpose
Heart Failure is currently the leading cause of 30-day hospital readmission in the United States, with readmission rates for Medicare patients hovering at 21.9%, potentially indicative of failures in transitions of care. Health systems which are able to implement comprehensive, multi-disciplinary cross-continuum transitions of care have previously been successful in reducing 30-day readmission rates. For the aforementioned reasons, there has been a growing interest in creating comprehensive, multidisciplinary heart failure clinics that are able to serve patients through the continuum of care and are financially sustainable. Despite this, there are few publicly available resources to guide individuals and institutions in establishing those clinics.
The HFSA Quality Care taskforce convened a working group with the express goal of creating this practical document and accompanying toolkit containing best practices to be utilized when establishing a successful heart failure clinic. As we work to finalize the development of the new toolkit the HFSA is conducting an environmental scan to gain a more granular sense of the practices of successful HF clinics throughout the United States in academic, hybrid, and private practice settings.
