  • The most important things healthcare providers can do to help patients w/ #HF take an active role in their treatment plan are:
    1. Ensure patients are on the best doses of the best meds 
    2. Educate them on the 4 pillars of guideline directed medical therapy #GDMT
  • Patients should understand that #GDMT can  

    Improve the heart function 
    Decrease risk of hospitalization 
    Improve symptoms and quality of life 
    Decrease mortality

  • It is also important for patients to understand that the higher we can get the doses of these medications, the more the effect on the heart, and thus, the greater the benefit! #HFChat2023
     

  • Lastly, providers should share with their patients a chart of the target doses of the 4 pillars of GDMT so that the patients can see where they are on their pathway to maximizing these medications. 💪 #HFChat2023
