It offers easy and quick access to prevention management tools, promotes the HF guidelines with simple and clear messages and helps to raise awareness on heart failure risk management.

This guide includes an overview of the numbers and stats that all patients should know such as how to monitor their symptoms and how to keep their fluid intake to a minimum; as well as checklists of discussion points for Tracking and Reporting Symptoms, Lifestyle, Medications. 

The overcoming barriers to change section of the Clinician's Guide: Taking With Your Patients About Heart Failure will help to reduce health disparities by shedding light on barriers that patients may face when attempting to make healthy changes. The tools and follow-up interventions are linguistically and culturally tailored and are available to persons with disabilities and other underserved populations.
