This guide includes an overview of the numbers and stats that all patients should know such as how to monitor their symptoms and how to keep their fluid intake to a minimum; as well as checklists of discussion points for Tracking and Reporting Symptoms, Lifestyle, Medications. 

The overcoming barriers to change section of the Clinician's Guide: Taking With Your Patients About Heart Failure will help to reduce health disparities by shedding light on barriers that patients may face when attempting to make healthy changes.
