The Heart Failure - The Basics: A Patient's Guide was developed to help patients understand heart failure risks, symptoms, treatments, detection of, and lifestyle changes to living with heart failure. The guide contains prevention management tools such as downloadable medication lists, instructions on how to read a food label, and instructional videos.
The guide ultimately seeks to improve health outcomes by identifying patients’ modifiable health risks and providing follow-up behavior change interventions.
