Redesigning Heart Failure Care Through Implementation Science
Evidence-Based Guidelines and Precision Medicine Approaches Promise to Transform Heart Failure Care, yet Significant Gaps Persist Between What We Know Works and What Happens in Routine Clinical Practice. This Session Will Detail Implementation Science Frameworks (KTA, CFIR, RE-AIM), Stakeholder Engagement Strategies, Measurement and Sustainability Approaches, and Workflow Integration. Drawing on Successful Implementation Models and Emerging Efforts in Heart Failure Genetic Testing Implementation Through REIMAGINE-HF, as Well as GDMT Initiation Clinics, Pharmacist-Led GDMT Initiation Programs, and the GDMT Polypill.
The Presentation Will Also Address the Scalability, Feasibility, and Cost-Effectiveness of Such Programs, the Role of Health Literacy, and Health Equity Considerations. Innovations in Research Efforts, Including the Role of Qualitative and Mixed Methods Research, Will Highlight Barriers and Facilitators to Implementation. Attendees Will Leave Equipped With Actionable Frameworks for Launching Their Own Implementation Initiatives.
