Lucianne “Lucy” West, PharmD, BCCP is a Cardiology Clinical Pharmacist with particular interest in advanced heart failure and population health. She graduated from Northeastern University, School of Pharmacy in 2016 and went on to pursue both her PGY-1 Pharmacy Residency and PGY-2 Cardiology Specialty Residency at The Johns Hopkins Hospital in Baltimore, MD. Prior to joining the Tufts Medical Center team, she practiced at the Cleveland Clinic in Cleveland, OH. Lucy’s clinical interests include medication optimization in chronic heart failure and heart transplant, improving transitions of care, and implementing innovative practices to increase access to pharmacist services. She also enjoys serving as a preceptor and mentor for student pharmacists and PGY-1 and PGY-2 pharmacy residents.  Lucy has served in professional association leadership roles at the national, regional, and state level, and is an active member of the Heart Failure Society of America, American Pharmacists Association, and American College of Cardiology.
