Heartfelt Hero Award

The purpose of this award is to recognize a heart failure patient or caregiver who, through their voice and actions in the heart failure community, demonstrates unwavering commitment to improving the lives of HF patients and inspires others on their journeys.

2024 Award Information

  • Award Presentation: HFSA Annual Scientific Meeting 2024 (September 27-30, 2024 | Atlanta, GA)
Eligibility
  • HFSA members will nominate patients or caregiver for the award
  • Nominee must be an HFSA patient/caregiver member (if not currently a member of HFSA, membership enrollment is complimentary for patients and caregivers)
  • Past winners of the award are not eligible 

Evaluation of nominees and selection of the recipient will be chosen by a majority vote of the Awards Committee and submitted to the HFSA Board of Directors for approval.

Nominations

Each nomination should consist of:

  • Previous nominees can be re-nominated (new submission is required) and posthumous individuals may be nominated
  • Cover letter including contact information of nominee (name, address, phone, and e-mail)
  • A summary of accomplishments addressing each of the evaluation criteria (maximum length one page)

Nominations may only be made by HFSA members in good standing. Self-nominations will not be accepted.
