Heart Failure & Transplant Team at Intermountain Medical Center was named the 2024 Outstanding Heart Failure Care Team Award winner.

The Heart Failure (HF) & Transplant program at Intermountain Medical Center (IMC) in Salt Lake City offers the full spectrum of HF care to patients in Utah and across the Intermountain West. As one of the four local area hospitals that collaborate to form the U.T.A.H. Cardiac Transplant Program, they have offered heart transplant since 1985. This multidisciplinary team consists of 80 team members including physicians who are Advanced HF & Transplant certified, cardiothoracic surgeons, Advanced Practice Providers, nurses, pharmacists and pharmacy technician, mechanical support nurses and the ECMO team, transplant assistants, medical assistants, schedulers, biomedical engineer research associates, genetic counselors, financial coordinator, social workers, a quality manager, dietitian and administrative support. The HF & Transplant program at IMC has some of the best transplant and LVAD outcomes in the nation, is a Hypertrophic Cardiomyopathy Center of Excellence and is recognized for excellence by Cigna and Optum and the U.S. World and News Report. The team maintains excellence through strong leadership support, regular team meetings, focus on team education and mentorship, quality improvement projects and dedication to patient education.  Over the years, this team has contributed to well over 100 clinical trials and numerous publications across HF, transplant and mechanical support.
