Heart Failure Heart Storylines (Updated!)

Long-awaited update for the Heart Failure Health Storylines App: It’s been over 6 months and we’ve come a long way thanks to all your feedback! We’re back with some big updates and we can’t wait ‘til you see them.

The Heart Failure Health Storylines app is created with input from people with heart failure and congestive heart failure (CHF) so that the right health tools are available to you. Now it’s easy to record your symptoms, vital signs, medications, and more.

 

Education Modules

The Heart Failure Society of America has prepared a series of educational modules designed to help patients learn to live successfully with heart failure. The advice in the modules can help heart failure patients feel better, stay out of the hospital and live longer

Newly Revised Modules
In addition to the brand new Module 11: Clinical Trials, the following Modules have been revised:

  • Module 4: Following your Treatment Plan and Dealing with your Symptoms
  • Module 7: Tips for Family & Friends
  • Module 10: Heart Rhythm Problems

Pan Foundation

Get Patients Help with Treatment

The Patient Access Network (PAN) Foundation has a patient assistance fund for Medicare patients being treated for heart failure. Qualifying patients will be eligible to receive up to $1,500 per year to help cover the out-of-pocket costs associated with their prescribed heart failure medications.

Eligibility Criteria

  • The patient must have Medicare health insurance that covers his or her qualifying medication or product.
  • The patient’s medication or product must be listed on PAN’s list of covered medications.
  • The patient’s income must fall at or below 500% of the Federal Poverty Level.
  • The patient must reside and receive treatment in the United States or U.S. territories. (U.S. citizenship is not a requirement)
