Center for Advanced Heart Failure and Mechanical Circulatory Support Team at Rochester Regional Health was named the 2024 Outstanding Heart Failure Care Team Award winner.

Our heart failure team comprises 5 physicians, 7 APPs, 4 nurses, a pharmacist, a social worker 3 MAs and 4 administrative assistants. We take care of thousands of patients throughout upstate New York, with many patients traveling 3+ hours to seek care. We work collaboratively with centers around upstate New York, making full use of remote monitoring, video and teleconference visits and partnering with interested cardiologists and primary care physicians to keep patients as close to home as possible. Our team and our patients have become family, and we strive to provide the best care we can in a geographically isolated and underserved region of New York State.
