Presenter Disclosure Information & Slide(s)

A disclosure slide is required for each oral presentation being given
for satellite programs and for the main scientific program.
This slide will be the first slide shown as part of the speaker's presentation.
Two template power point slides are provided in the following file. Please select the
template slide that is most appropriate for your presentation and modify accordingly.

If your presentation is part of a satellite program, the company providing
support for the satellite program will need to be identified in the bottom sentence (Slide #2).
Please remember:
- presentation should be balanced and use generic terms only
- off-label use(s) should be identified as such
- patient care recommendations should be supported by adequate evidence
- Relationships listed should include all companies with which
you or your spouse or partner have or have had a financial relationship in any
amount in the past 12 months and which provide products or services relevant to
the subject matter of your presentation.
