2008 HFSA Membership Application - Closed for 2008
Annual Dues
Annual Dues are $150.00 for Full Voting Membership and $35.00 Trainee
(full-time student, resident, post-doc) - verification letter required from
program director to include "full time" status and date training to be completed.

Completing the Membership Application
Once the application has been opened in Adobe Acrobat Reader, the form
can be completed using the "tab" key on your computer. Each time the
tab key is hit the cursor will advance to the next field requesting information.
Once all information has been entered, print the form immediately and fax to the
HFSA office at 651-642-1502.
